- Type 2 DM in patients inadequately controlled on diet, exercise, and monotherapy with either glimepiride or metformin:
- Initial dose: PO 1mg/500mg taken with the largest meal.
- Maintenance dose: Increases should be made in increments of 1mg/500mg per day, with at least one to two weeks between titrations.
- Max: 8mg/2000mg
Tablet:
- 1mg/500mg
- 2mg/500mg
- 2mg/1000mg
- Take orally with meals to reduce GI side effects (metformin) and the risk of hypoglycemia (glimepiride).
- Regular monitoring of blood glucose levels is essential, especially during dosage adjustment.
- Avoid excessive alcohol intake, as it may increase the risk of lactic acidosis (metformin).
- Glimepiride (Sulfonylurea): It stimulates pancreatic beta cells to secrete insulin, thereby reducing blood glucose levels.
- Metformin (Biguanide): It decreases hepatic glucose production, reduces intestinal glucose absorption and improves insulin sensitivity.
